I drew up this very rough draft of a consent document that lists acts that go against mine and/or your boundaries. Please feel free to review it, and request a downloadable form. Edit it in any way you want. Have it stored in a safe place, if needed, I will research ways to see if the members area of this website would be a safe place to have a registry of this kind. If you can, take it to your attorney to see if it meets all the legal requirements to protect you or your children from acts that you do not approve of. Everything that can be proven usually needs to be on a dated document, I hope this will help. At least It could be a witness that the form was requested, which could be considered another level of verification of wanting to establish protection from anyone going against your boundaries.
